You will find various forms and information leaflets to download from this page. If there is something you need which does not appear on this page, please contact Manchester Play on 0161 274 6519.

Resources Page 2 includes lots of useful proforma's such as Consent Forms, Registration Forms etc

Formats available are Word and PDF. Word can be submitted by email once you have saved your file and entered data. Email your completed forms to mark.fletcher@manchester.gov.uk. PDF versions need to be downloaded, printed and the form completed by hand. You will then need to mail your completed form in the usual way. If you need to download a free copy of Adobe PDF Reader, click here. Alternatively, in some cases you can complete a form online, just follow the link for instructions.
